JAN FORMALIZA
Born on January 14, 1984, 24 year old Jan Formaliza obviously took her name from the lunar month of January. There’s some sentimental attachment to the name, as her mother Jo Formaliza described it as secret. A soft-spoken, demure and simple lady, as her friend and co-worker described her, Jan is an epitome of a modern and yet conservative woman of the sixties that blooms in a modern day era.

Jan is the eldest daughter of Jocelyn and Ronnie with two younger brothers who are still both on their secondary education. She finishes her elementary at Naga Central School and continued her secondary at the Universidad de Santa Isabel. She finishes her college with a Bachelors Degree in Nursing and graduated in 2007. She passed the Board examinations that same year.

When asked about her interest in life, Jan straightforwardly told about her dream of having her own family in the future and be a good mother to her children. She manages to be a strict but composed sister to her younger brothers and made a promise of supporting them until they have finished their studies.

She first stepped into the limelight of advocacy and volunteerism in early first quarter of 2007.  Jan has been recommended by her ever supportive mother to work in the Blood Bank Section as JOW Blood Bank Nurse, replacing her fiancé, Vic Ibarreta, who went abroad for a job. By then, she started doing the tasked that her fiancé left behind her and that is being an advocate of voluntary blood donation.
